code on back, billing telephone number. $25.00 registration fee for one
student. $40.00 for family.
SUMMER CAMPS-Auditions will take place the first day
of camp. Students should prepare a 30 second song. (Optional)
We will be instructing on Auditions as well.
RULES AND
REGULATIONS
Parents are to bring students into the Studio for
lessons and come into the Studio to pick them up.
I understand that the Conservatory of Music and Dance does not take
responsibility for students before, during or after their class time.
Students will remain under our professional guidance only during the
assigned class time.
The
Conservatory of Music and Dance will not take responsibility for injuries
that occur at the School, or outside the school for outside functions
associated with the programs offered. There are many injuries that can occur
from movement and other activities in dance.
Any student with mental/physical problems must present a Doctors Note
permitting the student to partake in any classes in the School.
Verbal explanations and/or information about the student is not
acceptable. All information must
be in writing and signed by the student’s doctor.
The signature below acts as the guardian who will be responsible for
all parties attending the program before, during and after the classes,
rehearsals or performances.
The
Conservatory of Music and Dance reserves the right to withdraw a student
from any or all of the Recitals, without costume refunds.
Students are invited to partake in our Recital.
If the Teacher or Directress feels the student has missed too many
classes, has inappropriate conduct in class or is associated with any action
that interrupts or delays the progress of the class or any other student,
that student may be required to withdraw from the class and/or Recitals.
Costumes are not refundable.
Parents have the option of choosing the size of the costume, or have the
Directress choose the size of the costume (Teachers do not choose the size).
If the costume does not fit correctly, the parent must send it back
to the Studio within 5 working days of receiving the costume, with the
exchange fee. The fee must
accompany the costume. The
Conservatory of Music and Dance does not take
responsibility for sizing, we can only assist you by our experience
with growth. I give
permission to the Conservatory of Music and Dance to use pictures/videos of
my child for advertising purposes.
Names will not be associated with the pictures.
MANY OF OUR CLASSES FILL UP
QUICKLY, WE ARE VERY SORRY BUT IF CLASSES CLOSE WE CAN ONLY PLACE THE
STUDENTS IN AVAILABE CLASSES.
I have
read the rules and regulation of the school found on the opposite side of
this paper and understand and agree to follow the policies.
I understand that our handbook is online and must be reviewed as
well.
I
understand that communication will be posted on the website, through email
and the bulletin boards in the school.
Emails are sent out monthly, so if you do not receive them, please go
to our website above and fill out the email form.
Tuition should be paid at the first class of the month or session by
placing it in the black tuition box in the school or paying it with a Visa
or Mastercard through the office during office hours, or you may pay online
through our secured site.
A
registration fee of 25.00 per student or 40.00 per family must be received
for students to be placed in a class.

submitting the form and forwarding a fee, I have agreed with the school
rules and regulations above and the policies
in our handbook.

1 hour class:
$50.00 per month for one class per week. Tots Class: 45
minutes Comb- 42.00 per month
MUSIC LESSONS:
24.00 per class half
hour. 8 week sessions.

Please Note: If you register 48 hours or
less, before your class, you should wait for a confirmation before you attend.
There is a possibility that the class is not open or is filled. We are
updating the website daily and within a 48 hour time period of classes closing.
